What is the proper course of treatment or protocol when switching from 115mg of methadone to 8 mg of suboxone?
Dear Happyperson, welcome to our site. I do want to give you a bit of info you might not like, but it is true. NO ONE should attempt to switch from thatn high an amount of methadone to suboxone. It is done in rehab centers where they have medical personnel who can monitor and intervene in emergencies. The proper way to switch from methadone to SUBUTEX, not suboxone, is to be tapered to 30 mgs or less of methadone per day, 72 hours WITHOUT any methadone, then slowly start on 2 mgs of subutex, in a drs office. That high an amount of methadone will react with the suboxone, unless you go 2 or 3 WEEKS without methadone maybr, but that high an attempt switch could result not only in precipitated withdral, but also other medical emergency symptoms, that is why rehab or detox clinics only, handle switches from that high a dose. I am not trying to sound mean or judgemental, i hooe you can successfully be switched, but doing at home from that dose is way to risky for precip and other problems. Subutex, does NOT have naloxone and is the better med for a methadone patient. I am soo sorry i had to tell you this. We get similar posts, some people have tried this at home on their own,and they got very very sick, some had to be taken to the ER, please don't attempt this on your own. Best of luck, patti
