Dulera should work just as well as Advair. They both have two ingredients, one is a steroid and the other is a bronchodilator.
If Spiriva was helping you lots and you don't have that now, then you may experience some breathing issues but hopefully not too much. If you do then you might have to talk to your doctor or the insurance company again.
