I have been on coumadin for almost 6 years due to a mechanical aortic valve. To the best of my knowledge I have never missed my nightly dose, but if I did I would wait until time for the next dose. If I were to double-up it would mean taking 20-22mg at once. Do your best to not miss any doses; I have a pill box that holds seven days' worth of doses. Before that I just made sure I remembered.
