It sounds like you could be having anxiety attack's.. I've had countless anxiety attacks at bedtime and they produce physical symptoms that feel real enough to cause me to go the e.r.once i even called an ambulance!!! They never find anything wrong with me physically and its just amazing how powerful the mind is that it can give me physical symptoms of the nature you've described & many more very scary one's.I've gone as far as avoiding sleep altogether to avoid any of these troublesome feeling's which is never a good idea. I have sought help from a psychiatrist who was able to give me an antianxiolytic med to be taken at bedtime but i wouldn't encourage you to do that either due to the potential for addiction to that class of med and given you are so young.
You sound like you have an anxiety disorder, with or without panic attacks. Seeing a good GP (General Practitioner) or Internist may help. Best of all would be to see a good psychiatrist because s/he's the expert in knowing exactly what medicine you need and how much. This anxiety can vary through the years and different life pressures, so it's great to have a Dr. who knows what you need. Psychiatrists manage drugs, that is their specialty. Going to one doesn't mean you're crazy.
Anxiety can cause everything you wrote about and more. Why not start searching for a good Dr.? You probably need to be on a maintenance dose of something that will help you avoid anxiety attacks or an anxiety disorder. You may also be prescribed something to help when you feel an attack coming. Make sure it's a real anxiety attack or you'll take your pill and probably be so sleepy you can't function. Life is so much better when your problems are treated.
