Why is it when a pt leaves a nursing home their Rx are not transferable to a regular pharmacy?

The dr is supposed to give you prescriptions for whatever you need at home. They may be different from what was needed in the facility. The prescriptions in the facility do not have refills. And they were already filled.

In my mother's case the prescription cards all show multiple refills. The nursing home said we could transfer them, but when the pharmacist called the Rx provider they said no. She just relocated to another state to live with family and while we try to find a MD to take her she still needs refills.

Multiple refills for the Home to order but not necessarily for a regular pharmacy to fill. Some scripts may transfer and some may not, especially if going to another state. Controlled substances never transfer so any prescription pain meds, meds for "nerves" they wont transfer-you have to have written scripts.

Question, how do you know what's on the med cards? Did they give them to you when she left?

Yes, they gave us all her med cards when she left.

Regulations can vary from state to state. Always remember if you are going to go out of state, rules may be slightly different so always get written scripts. You should be able to get scripts from the medical director of the nursing home. They should have made sure she had scripts when she left. That is laziness or lack of knowledge on the part of the home.
