I want to be able to go to a family doctor and get what the ER gave me however I do not want to show up and just simply say this is what ER gave me and it worked. I would like to have proof.. However it has been sometime ago and I cannot for the life of me remember what doctor I had... Please HELP!
If you were in the ER, then the hospital has the record regardless of which doctor. Contact medical records and give them name and approximate dates and they can find them. You have to go to the hospital and sign a form to obtain though or have the hospital fax the form to you and you fax back.
Hello, don't worry! It's perfectly fine and in your favor, you HAD gone to the hospital for a legitimate claim and the dr there felt you needed klonopin. You should go request your medical records from the hospital and take them to your dr appt. basically, the problem you had 1-2-3-4-5 months ago has reoccurred and the medicine they gave me at the hospital really helped you and that's why you made an appointment today so you feel better or however it helps you tell him but don't be afraid. It's perfectly normal, and I have witnessed it over and over how people come to the ER and then they are started on medication. You thought you felt better and so you didn't come to the doctors to get it but now you need it
You could always go back to the e and start over then go to the dr shortly after as well
