And my doctor's office refused to give him it saying that it would be a hipaa violation to give him patient information. I thought pharmacists can ask/are allowed to know these thing. So now I have to go to my doctor's office and fill out a release form. Is it really a hipaa violation?
They are. The pharmacist is part of the healthcare team and therefore doesn't figure in the hipaa regs. Your dr is being an idiot. Particularly with a drug like adderall, he has to supply the necessary info if he wants his scripts filled. It should be on the prescription. Its the same as if I, as a nurse, was to take care of a patient. The dr has to supply me with the info needed to care for the patient.
