Dr. has asked my dad to rate pain on 1-10. Also, need to log meds taken in. Need an organized foolproof way to do this.
Yes, there are many online. Try the American Pain Foundations web site or another really good one is the American Chronic Pain Association. There is a section to click that says 'Pain management tools' then click 'communication tools'-these are very helpful for what you want. There is a pain log, daily activity checklist, medication and allergies card to list medications, a quality of life scale, and pain maps where he can draw on them to show exactly where he feels his pain and a few other tools you might find helpful. The American Pain Foundation has a search engine that brings up a lot of really good info and links to other web sites.
