Do nurses need a doctor's order with instructions to alternate PRN Tylenol Q6H and PRN?

Ibuprofen Q8H especially for a pediatric patient. I need a lawful answer, please. No opinions. Thanks.
If they both are ordered separately she should probably have an order that says may alternate. We used to be able to use our heads, but now they think we're stupid. Both meds should be the same time span too. That way the child would get a pill every 3 hrs.
