The first day you have red blood flow, not just brownish spotting, is day 1. You use USP progesterone cream from day 12 to 26, if you have a 28 day cycle. If you don't, stop the progesterone cream 1-2 days before the day that should be your next Day 1. If you are trying to get pregnant and on what should be Day 1, you don't start your period, start using the progesterone again, either until you get your period or have a positive pregnancy test. It is best to mark your calendar with Day 1 every month because a lot of people don't have 28-day cycles. Your previous cycle ends the day before the next period starts. You may have a very regular cycle and always start on the same day # or it may vary, but you will have a much better idea of when your period starts and when to use the cream.
