Not using back up when appropriate is one of the biggest reasons that birth control pills fail. There is a certain waiting time before the Pill kicks in to start working where one needs to use back up protection. Antibiotics reduce the effectiveness of oral birth control pills and back up, like condoms, should be used the whole time you are on antibiotics and for seven days after you finish. If you did not do this, then there is a big chance that you got pregnant. Birth control pills are not magic, you HAVE to use them properly as directed.
