Fentanyl can take up to 8-12 hours to become fully effective. Yes it is better than 5mg vicodin. Fentanyl is a derivitive of morphine and does work very well. Every person has a different reaction to any medication therefore what may work really well for someone else may not work as well for you. 25mcg is not the lowest available 12.5mcg is the lowest available but most doctors start with 25mcg and then increase it every two-four weeks depending on your level of pain. You need at least 2 weeks of continous use before you will know whether or not you will need to have the dose increased to 50mcg or higher. the ranges of fentanyl are 12.5mcg, 25mcg, 50mcg, and 100mcg. Some doctors will also use two patches if you need to go higher then 100mcg. Good Luck and I hope that the patch works well for you. It did for me until just recently when I was so tolerant to it that even increasing the dose didn't make a difference. Be very honest with your doctor and let him/her know if it isn't working well enough for you. Most people also need breakthrough medication when taking the patch for around the clock pain relief.
