Be very open and honest about why you think you need an increase. The doc will take in to account, how long you've been on your current medication, what strength it is already, type of injury or illness you have, when/if you had the last increase, how well they know you, if you ever gave them reason to suspect you are "seeking" drugs, a whole host of considerations will be on the doc's part. The best thing to do (depending on all I mentioned) is tell the doc what is happening and the meds you have aren't working as well. Honesty is the best policy.
