I hope someone who has done so successfully answers also, but I have considered how to do that also, and my dr. is a Mormon who knows my family but has stated on his promise to my patients letter that drug testing there will not include or be judged on pot consumption. I would just say, "I want to know your feelings on marijuana use to help alleviate my pain. I have heard a lot of talk about how effective it is and I have spent a lot of time investigating the pros and cons and think I would like to at least give it a try What do you think? Is that something you do here?" I would be serious, meek, sincere and look him/her in the eye. If he cuts you off and puts up an immediate wall, I would smile, thank him and just say you were just looking for another way to deal with your pain issues and knew he would be the one to ask so he feels you respect him/her. Then I would get going on helping get the law passed in Nov and ask around to see if you can find people who have that card and follow what they did to get it. Good luck.
It's really very simple. Your Dr. KNOWS your history w/pain, etc. You have a well established history w/your Dr. also. Basically it's just a matter of asking him/her if they would sign the form. Have it in hand when you go to your appt. My Dr. knew that the next step for me was narcotics & the side effects are much higher w/narcotics (proven medically & scientifically also.) My Dr. checked the chronic pain box & agreed that for me it would be better for me to have the medical marijuanna. Since I got my card in January I have had 80-90% less pain. No exaggeration.
