I recommend you seek an appointment with a dermatologist, not a family doctor, as they are more familiar with skin disorders, to secure a diagnosis with certainty. Should you have HS, ask the doctor if major surgery will be a problem in their opinion. They will have seen the condition of your active boils and will make a better-informed decision than anyone else. It is smart for you to be concerned. You could develop a fever if you have a bad boil, for example, and that could cancel the whole surgery. With a positive diagnosis, make sure you inform your orthopedic surgeon, and give them the name of your dermatologist so they can discuss your HS and whether it will interfere with the procedure from a staph/fever/septic/white-red-blood-cell-count perspective.
