Yes. My aunt got pregnant with her 3rd child when Mirena was still impanted in her uterus (it's likely it was implanted improperly, but she still got pregnant - and she is a doctor!).
No form of birth control is 100% effective, especially given human error and other environmental factors out of your control.
Mirena + condom seems a safe bet, but even with this combo, you COULD get pregnant (it's just VERY unlikely... odds would be something like 0.001% chance).
