Sounds like it could be some kind of parasite. I have a 20 years old goldfish that had anchor worms, which I treated with a medication called "Clout". This medication treats several diseases such as ick, anchor worm, fish lice, and skin flukes. The box does not mention anything about tapeworms, but if you go to www.happy-goldfish.com you can look at the pictures and descriptions of various goldfish diseases and treatments. If you have a local fish/pet specialty shop they might be able to help you if you describe the problem, or try to take a picture of your fish.
