There's not a medical professional out there who is going to advocate smoking regardless of the medication you are on. With that being said, I have been on 100 mcg fentanyl patches for nearly 10 years now, and I smoke about 1/2 a pack a day. I've had no complications with smoking and using fentanyl. However, since fentanyl can decrease your breathing, I'm sure someone with lung problems like asthma or the like should avoid smoking completely.
