Is the excelon patch considered an "over the counter" drug for purposes of Medicare D?

Lord no. If you're not able to get it paid for, call your customer service rep. You may need to get your plan changed, and now is the time to do that. Part d has many plans, and some won't pay for that. Mine only pays for generics, because so far I've managed to stay on all generics. It saves me allot of money that way.
