It is the original contract, and not a copy. There was no letter of explaination.
Why would my DR mail me my pain management contract?

I would FAX it to the office-that way you have proof that you sent it to them, or mail it certified or whatever to prove that you returned it. Also, a friendly phone call should be quick and easy. Call, identify yourself, and say in a funny joking tone, "Hi, somebody accidently mailed me my PM contract. What should I do with it?" They'll tell you. I got a letter last week from my doctor that I didn't do lab work in Dec. I called the lab-and confirmed that my PCP hadn't ordered any more, and the lab did everything he ordered. Just a mistake.
