I was started on the 50mcg patch this past Wed., and I was getting relief probably within 8-12 hours, but I was itching all over and also suffered with insomia. Much like you I too felt like I was bouncing off the walls. I can say that out of all the pain meds I've tryed the patch has been the best at relieving my pain... Good luck, hope it works for you too.
Definitely take the benedryl. I took some before I applied the patch and plan to continue taking it at regular intervals to off set the itching. As of right now my pain is slowly decreasing and I'm not itching near as bad. I can also say I feel like I could lay down for a nap. I have a 20 month old and she just went down for a nap and I'm going to try and do the same. I think you do need a higher dose and thats why I suggested putting 2 on, but thats your call. I also took some advice I recieved from others on this site and applied the patch to a fatter area ie: my lower stomach verses my chest due to the itching. If the itching wasn't an issue for you apply them on less fatty areas, your chest or arm for example. I feel for you and I wish you the best of luck. I have heard that fentanyl doesn't work as well for some people, maybe your a better canidate for oxycontin or morophine. I feel that most doctors feel that if fentanyl isn't working then nothing will because fentanyl is the strongest pain killer on the market, but what they fail to realize is that each person is different.
