Does medicine lose its effectiveness after taking it for several years?

I have been taking metformin for many years and recently ran out before going back for a refill. I can't tell any difference in my blood sugar from when I was taking it.
I take many medications and they ( what you experience, I do too) do not lose their effects-benefits, we develop tolerance to the meds, that is why every now and again doses need to be adjusted.-
I have heard that before also regarding diabetes medications.
Uncle of my friend was on antidiabetic drugs for 20 yrs with good outcomes.
But now his blood sugar has started to rise and is hindering a cataract surgery he has to undergo.
