Diagnosis of bronchiolitis obliterans with deteriorating lung function tests. Also the presence of several lung infections. Patient has been prescribed 4 days of antibiotics via PICC line. He is also prescribed 450mgs today and 750mgs of prednisolone tomorrow. These are huge doses of corticosteriods. We are worried, can you elaborate as to why so much, when uncontrolled lung infection is contraindicated. He has been on AB's for 6 mnths now to treat lung infection,so far unsuccessfully.Pt has NH Lymphoma and has had a bone marrow transplant and several rounds of chemo. He is currently neutropenic.Looking forward to your answer. Regards Julie
Are the worried about rejection of the bone marrow? It almost sounds like a last ditch effort. It is a really high dose. I would talk to the dr about your concerns, but I know some docs would just say I ordered it give it. You have to kind of get around that. A dr once insisted I give a pt with a pulse of 20 digoxin, I refused, he got irate, I handed him the keys to the medroom and told him if he wanted him to have it give it himself. He didn't do it.
