Pull out your medicaid card and look for the customer service or members services number on your card. Call this number and ask them. Each state and each carrier and even each participating physician group within a state may be different on what they cover. Your insurance provider is the only entity who will be able to tell you if this is a covered benefit under your plan and if there is any criteria to be met (prior authorization, step therapy etc)
