Hi, I have been looking up everything I can but cannot find some specific advise. I will be detoxing myself at home off of a daily .3-.4 heroin habit of a few solid months; I have done this in the past twice and done well with a couple suboxone or subutex just for the first 3 to 4 days, because I like getting the kick over with. I would still cut back dose every day, and still feel some withdrawl later but I do not want to take anything for more than 5-6 days. So this time I have 80mg of methadone plus 2 8mg subutex, that I aquired from around town. My question is, what dose of methadone to take, and is my plan of starting with methadone and going to subutex a good one? I was thinking 3 days of methadone or so, tapering each day, then go to like 6mg of subutex on day 4 and taper from there on...
Going from methadone to suboxone or subutex is tricky, both are long acting opiate replacements, and they both hang around on the brain receptor sites for a few days. Normally, a person who is on methadone has to be tapered to 30 mgs or less and wait 3 days before starting subutex or the risk of precipitated withdrawal is very high. If you are unfamiliar with precip, it is very very bad withdrawal, and it occurs because there is residual opiate on the receptor sites, the 2 meds will fight for the same space on that site, dropping both meds off of it, and precip will occur. I know people do want to try to do this at home and quickly, but getting over withdrawal is just part of the battle, and I would NOT advise you to use the methadone and then subutex, stick with the subutex or the methadone, but don't take both, even if you wait the 3 days . I am not judging you, I want you to be safe and comfortable and mixing meds like these 2 can be dangerous and also lots of problems, improperly used methadone can be FATAL. If you could do some sort of group or individual counseling, you will greatly increase your chance of staying clean, and provide yourself with support, which you need. Since methadone does tend to hang around on the receptor sites, you do need to wait, the full 72 hours before attempting to take the subutex, you will be very sorry if you don't. The brain neurotransmitters must reboot before you will get off either ( including the H) without withdrawal, that is just the way it is. No pill or potion will do this as fast as you would like to, I wish I could tell you differently. If you really want to get off H and stay clean, think about going to a subutex doctor and the counseling and you stand a good chance of that. You will be in my prayers. Patti
