I filled out the form on the web page. It gave me an ID number, but when I tried registering it online, it was not recognized. The monthly cost is $126 with my insurance. An Amitiza sponsored card would be nice
I had to call them to get the savings card. I couldn't find a phone number on the website, but there was a graphic with a picture of a sample card and it had a number on it so I called that. It worked; I got to a person and they got me registered. It did not, however, bring the cost to $0, but it was darn close (I paid $1.10) for a 30 day supply.
I recommend calling them; the website was a complete was of time.
