I am 5 weeks pregnant and have really bad shoulder pain, am using icy hot patches to help with the pain. dont want to harm the fetus.

Icy hot patches contain menthol and methyl salicylate. These substances have not been studied in pregnant women to know their effects on a fetus. While I doubt that it would harm the baby, it is best if you discuss with your doctor before applying this product to your body while pregnant.
