Hepatitis B vaccine has been assigned to pregnancy category C by the FDA. Animal studies have not been reported. There are no controlled data in human pregnancy. The vaccine is only recommended for use during pregnancy when clearly indicated. The Advisory Committee on Immunization Practices recommends vaccination of pregnant women at risk for hepatitis B infection.

There are no data on the excretion of hepatitis B vaccine into human milk. The manufacturer recommends that caution be used when administering the vaccine to nursing women. The U.S. Centers for Disease Control and Prevention (CDC) consider the administration of inactivated virus vaccines compatible with breast-feeding.
