Psychotic disorders (treatment)—Olanzapine is used to treat the manifestations of psychotic disorders {01}. The effectiveness of olanzapine therapy for more than 6 weeks has not been evaluated in controlled trials {01}.

Mechanism of action/Effect:

The exact mechanism by which olanzapine exerts its antipsychotic effect is unknown {01}. However, this effect may be mediated through a combination of dopamine and serotonin 5-HT 2 antagonism {01}. Olanzapine is a selective monoaminergic antagonist with a strong affinity for serotonin 5-HT 2A and 5-HT 2C receptors, and dopamine D 1, D 2, D 3, and D 4 receptors {01}.

Olanzapine binds weakly to gamma-aminobutyric acid type A (GABA A), benzodiazepine (BZD), and beta-adrenergic receptors {01}.

Olanzapine's high affinity binding to, and antagonism of, muscarinic M 1, M 2, M 3, M 4, and M 5 receptors may explain its anticholinergic effects {01}. Olanzapine also binds with high affinity to histamine H 1 and alpha 1-adrenergic receptors {01}. Antagonism of histamine H 1 and alpha 1-adrenergic receptors may be responsible for the occurrence of somnolence and orthostatic hypotension, respectively, seen with olanzapine use {01}.

Other actions/effects:

A modest elevation in prolactin levels persists during chronic olanzapine administration {01}, probably due to antagonism of dopamine D 2 receptors {01}. The clinical significance of elevated prolactin levels is unknown for most patients {01}, although such effects as galactorrhea, amenorrhea, gynecomastia, and impotence have been reported in patients receiving prolactin-elevating medications {01}. Also, studies have found approximately one third of human breast cancers to be prolactin-dependent in vitro .


Well absorbed {01}; however, 40% of the absorbed drug is metabolized before reaching systemic circulation {01}. The rate and extent of olanzapine absorption are unaffected by food {01}. Oral bioavailability of olanzapine was not affected by single doses of cimetidine (800 mg) {01} or aluminum- and magnesium-containing antacids {01}.


Extensively distributed throughout the body {01}, with a volume of distribution of approximately 1000 L {01}.

Protein binding:

High (93% {01}); primarily to albumin and alpha 1-acid glycoprotein {01}.


Olanzapine is metabolized primarily through oxidation mediated by cytochrome P450 (CYP) enzymes {01} and by direct glucuronidation {01}. In vitro studies suggest that oxidation is mediated by cytochrome P450 isozymes IA 2 (CYP1A2) and IID 6 (CYP2D6) {01}, and by the flavin-containing monooxygenase system {01}. However, studies in subjects who are deficient in CYP2D6 indicate that CYP2D6-mediated metabolism is a minor pathway of olanzapine metabolism {01}.

The two major metabolites, 10- N-glucuronide and 4´- N-desmethyl olanzapine, are not pharmacologically active at the plasma levels achieved during normal therapeutic olanzapine dosing {01}.

A study in six patients with clinically significant cirrhosis revealed little effect of hepatic function impairment on the pharmacokinetics of olanzapine {01}.



Mean, 30 hours {01}; range, 21 to 54 hours {01}. Mean apparent plasma clearance is 25 L per hour (L/hr) {01}; range, 12 to 47 L/hr {01}.

Time to peak concentration:

Peak plasma concentration of olanzapine occurs approximately 6 hours following oral administration {01}. Kinetics are linear over the therapeutic dosage range {01}.

Time to steady-state plasma concentration

Steady-state plasma concentration of olanzapine, which is approximately twice the concentration seen after a single dose {01}, is achieved in about 1 week with once-a-day dosing {01}.


        Approximately 57% of an administered dose {01} is renally excreted, 7% as unchanged drug {01}. Pharmacokinetics of olanzapine were similar in patients with severe renal function impairment and in patients with normal renal function {01}. Pharmacokinetics of the metabolites of olanzapine were not studied in patients with renal function impairment {01}.

        Approximately 30% of an administered dose {01}.

In dialysis—
        Olanzapine is not removed by dialysis {01}.

Precautions to Consider


In carcinogenicity studies, significant increases in the incidence of mammary gland adenomas and adenocarcinomas occurred in female mice receiving 0.5 times the maximum recommended human daily dose (MRHD) of 20 mg per day {02} of olanzapine on a mg per square meter of body surface area (mg/m 2) basis {01} and in female rats receiving two times the MRHD of olanzapine on a mg/m 2 basis {01}. A toxicity study in rats showed prolactin levels to be elevated up to fourfold at the same doses of olanzapine that were used in the carcinogenicity studies {01}. The increased incidence of mammary gland neoplasms found in rodents after chronic administration of antipsychotic drugs is considered to be prolactin mediated {01}. Drugs that antagonize dopamine D 2 receptors, including olanzapine, are associated with increased prolactin levels in humans {01}. Because studies have found approximately one third of human breast cancers to be prolactin-dependent in vitro {01}, this prolactin level elevation may be of importance when considering use of these medications in patients with previously detected breast cancers {01}. However, there has been no association shown between chronic administration of medications that elevate prolactin levels and tumorigenesis in either epidemiological or clinical studies to date {01}. Current evidence is too limited to be conclusive {01}.

Two carcinogenicity studies were conducted in which mice received olanzapine for 78 weeks at doses ranging from 0.06 to 5 times the MRHD on a mg/m 2 basis (0.8 to 5 times the MRHD in one study, and 0.06 to 2 times the MRHD in the other study) {01}. In one of these studies, a significant increase in the incidence of liver hemangiomas and hemangiosarcomas was seen in female mice dosed at two times the MRHD {01}. In the other study, this increased incidence of liver hemangiomas and hemangiosarcomas was not seen, but early mortality was increased in male mice receiving five times the MRHD {01}.


Olanzapine demonstrated no mutagenic potential in the following tests: Ames reverse mutation test, in vivo micronucleus test in mice, the chromosomal aberration test in Chinese hamster ovary cells, unscheduled DNA synthesis test in rat hepatocytes, induction of forward mutation test in mouse lymphoma cells, or in vivo sister chromatid exchange test in bone marrow of Chinese hamsters {01}.

The mating performance, but not the fertility, of male rats was impaired during administration of olanzapine at doses that were 11 times the maximum recommended human daily dose (MRHD) of 20 mg per day {02} on a mg per square meter of body surface area (mg/m 2) basis {01}. The impairment of mating performance was reversed with discontinuation of olanzapine administration {01}. Studies in female rats indicate that olanzapine may produce a delay in ovulation {01}. When olanzapine was administered at doses that were 1.5 times the MRHD on a mg/m 2 basis, female rats showed a decrease in fertility {01}. At doses that were 2.5 times the MRHD on a mg/m 2 basis, female rats showed an increased precoital period, and a reduced mating index {01}.

Adequate and well-controlled studies in humans have not been done {01}.

Of seven pregnancies that occurred during clinical trials with olanzapine, two resulted in normal births, one resulted in neonatal death due to a cardiovascular defect, three ended in therapeutic abortions, and one ended in spontaneous abortion {01}.

Olanzapine crosses the placenta in rats {01}. No evidence of teratogenicity was seen in rats administered olanzapine at doses up to nine times the MRHD on a mg/m 2 basis {01}, or in rabbits administered olanzapine at doses up to 30 times the MRHD on a mg/m 2 basis {01}. At the maximum doses in these studies, early fetal resorptions and increased numbers of nonviable fetuses were observed in rats {01}, and increased fetal resorptions and decreased fetal weight were observed in rabbits {01}. The maximum dose used in the rabbit study was considered to be maternally toxic {01}. Also, in rats, gestation was prolonged at a dose that was five times the MRHD on a mg/m 2 basis {01}.

FDA Pregnancy Category C {01}.

Labor and delivery—

The effect of olanzapine on labor and delivery in humans is unknown {01}. Olanzapine did not affect parturition in rats {01}.


It is not known whether olanzapine is distributed into human breast milk {01}. However, olanzapine is distributed into the milk of rats {01}, and use in nursing mothers is not recommended {01}.


No information is available on the relationship of age to the effects of olanzapine in pediatric patients. Safety and efficacy have not been established in patients up to 18 years of age {01}.


No geriatrics-specific problems that would limit the usefulness of olanzapine in the elderly were seen in studies that included elderly subjects {01}. However, the mean elimination half-life was found to be about 1.5 times greater in elderly subjects than in younger subjects in one study {01}.


Olanzapine clearance is approximately 30% lower in females than in males {01}. However, no differences in adverse effects or efficacy were seen in clinical studies {01}, and dosage adjustments based on gender are not recommended {01}.

Comparisons of pharmacokinetic data from studies conducted in Japan with data from studies conducted in the U.S. indicate a twofold higher exposure to olanzapine in Japanese subjects when doses are equivalent {01}. However, no clinically significant differences in safety or efficacy were seen when comparisons were made among Caucasian, African-descent, and pooled Asian and Hispanic patient groups {01}. Dosage adjustments based on race are not recommended {01}.

Drug interactions and/or related problems

Note: In vitro studies indicate that olanzapine has little potential to inhibit CYP1A2, CYP2C9, CYP2C19, CYP2D6, or CYP3A {01}. Therefore, olanzapine is not expected to interfere with the metabolism of medications that are metabolized by these enzymes {01}.
Combinations containing any of the following medications, depending on the amount present, may also interact with this medication.

Agents that induce CYP1A2 {01} or glucuronyl transferase enzymes {01} , such as carbamazepine {01} , omeprazole {01} , or rifampin {01}    (olanzapine clearance may be increased {01}; carbamazepine therapy at a dose of 200 mg two times a day increased olanzapine clearance by about 50% {01})

Agents that inhibit CYP1A2 {01} , such as fluvoxamine {01}    (olanzapine clearance may be decreased {01}, although, because multiple enzymes are involved in olanzapine metabolism, the effect of inhibiting one isozyme may not be significant {01})

Patient monitoring

Side/Adverse Effects

Note: Disturbances of body temperature regulation have been associated with use of other antipsychotic agents {01}. Caution is advised in administering olanzapine to patients who will be experiencing conditions that may contribute to an elevation in core body temperature {01}, such as strenuous exercise {01}, exposure to extreme heat {01}, or dehydration {01}.
The neuroleptic malignant syndrome (NMS) has been associated with the use of other antipsychotic agents {01}. NMS is a potentially fatal symptom complex that may include hyperpyrexia, muscle rigidity, altered mental status, and autonomic instability seen as irregular pulse or blood pressure, tachycardia, diaphoresis, and cardiac dysrhythmia {01}. Elevated creatine kinase, myoglobinuria (rhabdomyolysis), and acute renal failure also may occur {01}. Differential diagnosis should exclude serious medical illnesses, such as pneumonia or systemic infection presenting in conjunction with extrapyramidal effects, as well as central anticholinergic toxicity, heatstroke, drug fever, and primary CNS pathology {01}.

Note: Tardive dyskinesia occurs more frequently in elderly patients, especially elderly women {01}. The risk of developing the syndrome, and of experiencing irreversible effects, appears to increase with treatment duration and total cumulative dose {01}, although it may develop at any time during antipsychotic therapy {01}. There is no known treatment for tardive dyskinesia {01}, although partial or complete remission may occur when the antipsychotic medication is withdrawn {01}. Alternatively, the antipsychotic medication may suppress the signs of the syndrome {01}, masking the underlying process {01}. For these reasons, olanzapine should be used only in those patients with chronic illness that is responsive to antipsychotic medication {01}, and for whom potentially less harmful treatments are unavailable or inappropriate {01}. Also, the smallest effective dose of olanzapine should be used and the need for continuing treatment should be assessed periodically {01}.

During premarketing long-term continuation treatment with olanzapine (median exposure 238 days), 56% of patients had weight gain > 7% of their baseline weight {01}. The average weight gain was 5.4 kg {01}.

Clinical effects of overdose
The following effects have been selected on the basis of their potential clinical significance (possible signs and symptoms in parentheses where appropriate)—not necessarily inclusive:

Note: During premarketing trials, 67 cases of acute overdosage with olanzapine were identified {01}. The highest reported ingestion was 300 mg {01}. The only symptoms reported in this patient were drowsiness and slurred speech {01}. Among overdose patients who were evaluated in hospitals, none showed changes in laboratory analyses or electrocardiograms (ECG), and vital signs were usually within normal limits {01}.

Treatment of overdose
Multiple drug involvement should be considered {01}. There is no specific antidote to olanzapine {01}.

To decrease absorption—Gastric lavage, after intubation if patient is unconscious, and administration of activated charcoal with a laxative should be considered {01}. Activated charcoal has been shown to reduce absorption of olanzapine {01}, and may be of use since olanzapine does not reach peak plasma levels for approximately 6 hours following ingestion {01}. The risk of aspiration with induction of emesis may be increased by possible obtundation, seizures, or dystonic reaction of the head and neck {01}.

Specific treatment—Hypotension and circulatory collapse may be treated with intravenous fluids and/or sympathomimetic agents {01}. Because of olanzapine-induced alpha blockade, sympathomimetics with beta agonist activity, such as epinephrine and dopamine, may worsen hypotension and should not be used {01}.

Monitoring—Continuous ECG monitoring should be employed to detect possible arrhythmias {01}. Close medical supervision should continue until patient recovers {01}.

Supportive care—Airway should be established and maintained to ensure adequate oxygenation and ventilation {01}. Patients in whom intentional overdose is confirmed or suspected should be referred for psychiatric consultation.

Note: Olanzapine is not removed by dialysis {01}.

Side/adverse effects
Signs of potential side effects, especially agitation, akathesia, extrapyramidal effects, personality disorder, chest pain, fever, flu-like symptoms, mood or mental changes, peripheral edema, tardive dyskinesia, dyspnea, facial edema, menstrual changes, skin rash, and water intoxication

General Dosing Information
Since the possibility of suicide is inherent in schizophrenia, patients should not have access to large quantities of olanzapine {01}. To reduce the risk of overdose, the patient should be supplied with the smallest quantity of medication necessary for satisfactory patient management.

Olanzapine may be taken without regard to food {01}.

For treatment of adverse effects
Neuroleptic malignant syndrome (NMS)—Recommended treatment consists of the following:
   • Discontinuing olanzapine and other drugs not essential to current therapy {01}.
   • Providing intensive symptomatic treatment and medical monitoring {01}.
   • Treating any concomitant serious medical problems for which specific treatments are available {01}.
   • After recovery, giving careful consideration to the reintroduction of antipsychotic drug therapy in patients with severe psychosis requiring treatment, because of possible recurrence of NMS {01}; closely monitoring patients in whom antipsychotic drug therapy is reintroduced after recovery from NMS {01}.
Tardive dyskinesia—There is no known effective treatment {01}. If signs and symptoms of tardive dyskinesia appear, discontinuation of olanzapine treatment should be considered if clinically feasible {01}. To minimize the occurrence of tardive dyskinesia, chronic antipsychotic treatment should be in the smallest effective dose for the shortest duration necessary to produce a satisfactory clinical response {01}.

Oral Dosage Forms


Usual adult dose
Oral, initially 5 to 10 mg once a day {01}, with a target dose of 10 mg once a day within several days {01}. Dosage may then be adjusted as needed and tolerated at increments or decrements of 5 mg a day {01}, at intervals of not less than one week {01}.

While individual factors that decrease olanzapine clearance do not necessitate dosage reduction {01}, patients exhibiting a combination of these factors, such as an elderly female nonsmoker, should begin therapy with an initial dosage of 5 mg a day {01}. Dosage should be increased cautiously in these patients if clinically necessary {01}.
Debilitated patients {01}, patients predisposed to hypotensive reactions {01}, and patients who may be more pharmacodynamically sensitive to olanzapine {01} should begin therapy with an initial dosage of 5 mg a day {01}.
