Ipratropium competes with acetylcholine at the muscarinic receptors on iris sphincter to reduce the angle between the ciliary body and the lens so that the drainage of aqueous humor is reduced, leading to increased intraocular pressure, thus it is contraindicated in narrow-angle glaucoma.

In benign prostatic hypertrophy, the patient would experience difficulty in passing out urine. Ipratropium, as an antimuscarinic agent, can cause urinary retention which is esp. troublesome in pt with BPH.
