Patient Assistance Program Letter

I am new to the Patient Assistance Program and I am wanting to help people with their prescriptions that can not afford them. I have a senior that is on Medicare and I am wanting to get them approved with a Pharmaceutical company that does not except people with Medicare. Does someone have a copy of a letter that asks for an exception to their rule on approving their prescription for the Patient Assistance Program. I would really appreciate this. Like I said I have just started trying to help people. If there is any difference I have another that has Medicare and Part D. Thanks for any help.

There is a proggram through rx outreach that allows you to buy a lot of different meds at usually $20 per 6 month supply. The income limit is $54,000 a year for family of 2.
