Default med administration in the elderly

I'm wondering if anyone has a good idea about how to administer 30+ medications to an elderly person? I am a student, and my patient is taking around 30 medications throughout the day. I have never given one person that many medications at one time, nor have I given them to a person who may or may not be confused at any given time. I have the help of my instructor, but was curious to see others' ideas about the best way to approach this...
