Well the difference is in the time release, if you swallow it the way you are supposed to, the time release slowly releases the dose over a 12 hour period if you are taking
Oxycontin. However, if you crush it to snort, or chew it up to defeat the time released coating, you get all the dose released at once. So yes, of course then you would get the "rush" that drug abusers are so fond of finding.
The obverse, of course, is you put yourself in danger by doing this, if you are not used to using narcotics, and you suddenly take a strong dose of a strong narcotic like Oxy, you could end up killing yourself or putting yourself in a coma.
Then of course, there are the other health problems you can cause yourself, even if you are used to using narcotics. Almost all tablet forms of meds contain fillers and binders, the inactive ingredients that are used to make the pill a certain size and shape, and get it to set up in a solid form. Many of these, especially in the time released pills, are not meant to be used by our bodies at all, they are normally expelled in your bodily waste. That is why, on most of these meds, you will see little notes in the patient leaflets that say you might see the pill in your bowel movements, but not to worry, this is just the part your body could not use, the med itself has already been ingested and used as it was supposed to be.
These binders and fillers, being that they aren't digestible can cause blockages in your sinuses, throat, lungs, and etc. As a matter of fact, on the HBO Autopsy show, Ask Dr. Baden, a death caused by this was just covered a few weeks ago. A young man died, and everyone thought it was either suicide or a drug overdose. When he investigated and did the autopsy, Dr. Baden discovered that the man had been crushing and snorting whatever time released pills he could get ahold of to get high. He slowly blocked the little spots in his lungs that normally absorbed Oxygen, which our bodies need to survive. When he had snorted his last pill, the inability of his lungs to get enough Oxygen combined with the respiratory depression that these potent narcotics also caused and he smothered to death, with nothing else blocking his breathing passages, in his own bed.
So I don't think participating in this kind of behavior is worth the risk.
