Default need NDC for Zoladex

My insurance company needs the national drug code for Zoladex 10.8 mg injection(which I am taking every 3 months)
I am an American living in Venezuela.My doctor here is not familiar with the code .
If someone knows.Thanks in advance.

The complete number for the National Drug Code for zoladex injections 10.8mg taken every three months is 00310 - 0961 - 90. That number is good for the three month injections only. The firm name is Astrazeneca Pharmaceuticals LP if you need that as well. Good luck and God bless.

Thank you, Robert for your reply.Someone advised to look into the web of Astrazeneca before I read your input.I found the code is 0310-0951-30 for the 10.8 mg,3 months(90 days) inplant.
Now I am confused,is the last digit have something to do with the days?
If yes,where do you find your code?
I do not want to give my insurance company the wrong code,so they have the excuse to not to pay.
