Its a form you must sign stating you will comply with the terms / rules that the doctor lays out for you follow. Usually they are rules stating you will use only one pharmacy, get narcotics ONLY from him/her. If the rules are broken, they will more than likely cut off their relationship with you.

    Smile a pain contract

an agreement between you and your MD that you will only get meds from him and no other doctor or anyone. you will not share your meds and take only as prescribed. you cant call and say you accidently dropped them down the sink or whatever as you will get a certain amount each month and can only get the rx within a day or two of the refill date. you must agree to only use one pharmacy and not go to any other pharmacy without calling and telling your doctor you are changing pharmacy. it is standard for anyone on chronic pain meds to sign one of these forms. most all doctors do this.

Narcotic contracts are their way of telling you this is serious business. Trips to the ER generally "red-flag" a doctor that a patient may be seeking out more drugs. That's something many addicts do.

As for being referred to a specialist, ASK YOUR PRIMARY CARE DOCTOR about who does he want to prescribe pain meds - him or the specialist? The contract is a way to keep everything above board, with no way to get prescriptions for pain meds outside of what your primary doctor is 100% aware of.

IF you have any questions, ask your primary care doctor - in advance! Excuses later for why you needed to get a prescription elsewhere "this one time" won't hold much weight. Any violation of the contract can result in his refusal to prescribe any further narcotics to you.

These contracts are quite common nowadays, whenever a patient is going to be routinely taking narcotics for pain.
