Can you help me get off Fentanyl????

Can I please get somebody's opinion on how to get off of the Fentanyl Duragesic 25mg Transdermal Patch with as few side effects as possible? I am on this medication due to chronic pain in the lower portion of my body which I have because I have Friedreich's Ataxia. I would really like some kind of help. I want to be free of this medication.
