Well I will give a little background and then post my questions/ thoughts. I discovered my brother (26) was addicted to  <a href='http://www.drugs.com/oxycontin.html' target='_blank'>oxycontin</a> almost 2 years ago. Since I found out I convinced my parents to put him on  <a href='http://www.drugs.com/suboxone.html' target='_blank'>suboxone</a> instead of rehab. Not sure if that was a good decisions based on how this has gone. Anyway I live in Kansas City and he lives in memphis, so as far as I knew he was doing pretty good on the suboxone, but recenty had a breakdown  among many other things. To make the story short after talking to him I found out that his main problem is that suboxone controls his life and he feels like ******** a lot of the time becuase of it. He started at 16 or 24 mg im not exactly sure and he moved to Kansas City 6 days ago. He really trusts me and the plan was that I would completely control his medication and give him what he needs while weening him down because he wouldnt do it on his own. He was in agreement.  So he has been here for 6 days and every day is different some bad some good some real bad. BUT i should mention he told me he was taking 4mg 3 times a day until he got here. Since he has been here I have given him 4mg twice a day. He recently got a script of  <a href='http://www.drugs.com/xanax.html' target='_blank'>xanax</a>, but I feel like thats another addiction in the making. All he wants to do is take xanax becuase he says he can take a lot less suboxone when he has xanax. Anyways he only had a small script and is out with a little more on the way. He says it only helps if he takes up to 1mg. he is only perscriped .25 or .5. He also has  <a href='http://www.drugs.com/klonopin.html' target='_blank'>klonopin</a> which I give him sometimes when he doesnt have xanax and he says it doesnt really help as much but I definitely notice a difference in his mood. He seemed aboslutely fine yesterday and then today I gave him exact same amount of medicine, and after I got home from work (4 oclock everydyay) he was drunk as a skunk. He turns to alchohol when he claims he is in pain. He tells me he starts to have withdrawals after only 4 or 5 hours of taking the sub. I find it hard to believe after all the forums I have read. Anyways my big question is.... for the times when you decide to make a jump to a lower dose. How bad are symtoms? How many days will they last or does it take to adjust? What can be taken to ease this (xanax klonipin)? is it okay for him to drink or get drunk? I know I dont want it to be a habit and I wont let him but sometimes is that an appropriate way to deal with his pain. Obviously I cant completely understand because its not me and I wish there was a way without using other drugs but he seems to think xanax is they key! Any information would be very very helpful! Also I am keeping a day to day journal of doses of everything and how he feels and what happens that I may share after a while if he says its okay! <br />

Let me give you the short version of my story: Three yrs ago I was in a bad accident. I was given Fentyl patches for pain...I hated them and asked to be taken off....Docs put me on 80mg/day of  <a href='http://www.drugs.com/methadone.html' target='_blank'>Methadone</a>. Without any help from my doc I tapered down to 10mg/day by the end of Sept 09. Then made the jump over to 4mg subutex/day with success. I felt great again. At the end of week one on Sub I tapered to 3mg/day....felt great. End of week two I tapered to 2mg/day....felt great. After that I have been decreasing steadily every week since. Two weeks ago I was just below 1mg and had an awful week...no energy...little sleep...yawning...nervous. Then by the weekend I was fine. Last week, cut down to about .6mg/day....tired tired....yawning...nervous...hard to sleep at night. Then this past weekend I felt great. Last night I slept like ******** and woke up early. Didn't feel bad getting outta bed. Had coffee....a cig....read....showered and now I'm at work. Not feeling bad. Maybe a little tingley....not tired yet. Here's the deal. I didn't take my sub this morning. I'm on the fence. Do I skip days this week...then jump off starting next week and have a ********ty Turkey day? Or...do I just stop now and take some time off work this week and maybe get to enjoy my thanksgiving. My last dose was at 6am Sunday. I took .4mg <br />
I have a ton of vitamins at my disposal...Tyrosine, B Vits, Iron, Zinc..and multi vitamins...I have a clonazapam Rx...and some muscle relaxers (I have never taken either)...I hate the way any med makes me feel. Hence, the quitting of opiates. I hate them. I don't like pills. Never have. I don't like being drunk. I hate Pot. So...I don't like being messed up. I was on these meds because I couldn't walk and had to have relief. Had I known what i was in for, I would have taken the pain. I tried quitting methadone at 20mg...after about two days went into w/d.....it sucked. Not the worst I've ever felt (my nerve pain was worse after the accident). So....I can grit my teeth and take it. I am so ********ing sick of meds and docs that I am angry. I had told myseld I would jump next week. But waking up at 5am pissed me off and I am ready to take this bastard head on if I have to. Advice. Right now I am wondering what the &quot;best&quot; choice is.</div>

1. Is Ultram a Narcotic? Or NOT? Seems a lot say no, but then I also read so many W/D stories.......If it is NOT an Opiate/Narcotic, what exactly, would you get &quot;addicted&quot; to?<br />

2. And why does a &quot;high/euphoric&quot; feeling happen to about 50% of Patients......if it's not an Opiate....<br />

3. Lastly, I heard somewhere that you can't take Opiates while on Ultram, I heard that puts you into immediate W/D. ? ? <br />

I just wanted to know this so that I can better understand what people are posting about...because as far as this Medication goes...I am clueless.<br />

He's been on for about 4 years and doesn't use any other drugs, and has not used any other drugs for many years. He just wants off of the buprenorphine.<br />

<b>(I should also note: the biggest concern is that they know methadone and THAT THEY SEDATE YOU SERIOUSLY FOR THE WORST OF IT! Please, let me know -- even if it was for another opiate -- the medical detoxes that BREAK OUT THE HEAVY SEDATIVES!!!)</b><br />

Switching to buprenorphine -- at least outpatient -- is just not going to (i.e., has not and will not) work. Waiting long enough / switching to another opiate in order to induce was simply not tolerated. <br />
<br />
Tapering down off the methadone (and probably the buprenorphine) outpatient will take quite a while, I think, and will likely be worse than just doing a medical detox.<br />
<br />
Has anyone done a medical detox from 40 mg. of methadone, or a comparable  <a href='http://www.drugs.com/oxycodone.html' target='_blank'>oxycodone</a>/morphine/whatever dependency (e.g., 300 mg.  <a href='http://www.drugs.com/morphine.html' target='_blank'>morphine</a>/200+ oxycodone per day)?<br />
<br />
Does anyone have a location they recommend for accomplishing this?<br />
<br />
<b>If anyone has done this successfully, PLEASE let me know where you went and <i>how</i> it went!</b><br />

We're looking at several places. We cannot afford the ultra-expensive places, but they're all expensive, and I think that I can deal with that, especially when one considers the cost of NOT doing this, and doing it right.<br />
<br />
Medical Detox #1: Says they'll switch you from 40 mg. methadone onto oxycodone for 2-3 days, and then switch you onto buprenorphine, and then taper the buprenorphine down to 2 mg over 7 days and then give you the boot. All told, 10 days (or less, they say) spent. I have repeatedly heard that 2 mg of buprenorphine has as much (mu, at least) activity as 32 mg, and that the buprenorphine is equivalent to about 30 mg methadone -- so is this going to accomplish nothing but the equivalent of switching from 40 mg of methadone to 30 mg of methadone, only to face the withdrawals from the (long half-life-having) buprenorphine after leaving?!?!?<br />
<br />
If using buprenorphine for a week long detox has the magical properties it is said to have in some circles, please let me know. Otherwise, I am suspicious that once leaving this place one would simply go through pretty serious buprenorphine withdrawals. Or, are the withdrawals from 2 mg of buprenorphine NOWHERE near as bad as the withdrawals from 30 mg of methadone?!?<br />
<br />
Basically, for rehab #1, the question is: Can a 10 day detox using buprenorphine tapered down to 2 mg then going cold get someone off of 40 mg of methadone and not leave them feeling like total garbage upon leaving?<br />
<br />
<br />
Medical Detox #2: Uses buprenorphine like #1. Gets you on the buprenorphine after a switch to oxycodone, like #1. Has you spend about ~5-7 days to get on the buprenorphine in the medical detox setting, and then transfers you to a &quot;rehab&quot; type place where you go to groups and that stuff for another 3 weeks while the buprenorphine is tapered.<br />
<br />
All told, you switch to buprenorphine, and then have 3 weeks to taper off of it while in their rehab setting. I didn't find out how low they taper -- I assume it'll be either 2 mg or 1 mg.<br />
<br />
Is there a lot of value of extending the buprenorphine taper to about 3 weeks in a non-medical setting, expected to go to groups and stuff we're not interested in, instead of just switching and tapering over a total of 10 days in a more medical setting?<br />
<br />
<br />
Medical Detox #3: Does NOT use buprenorphine. Switches you to an unnamed short-acting opiate during the first few days. Then tapers you off of this drug during the rest of your stay. They estimated 21-28 days for a methadone patient.<br />
<br />
<br />
<br />
So, what are your thoughts? All have similar prices, and are (hopefully) feasible.<br />
<br />
Is the 10 day detox reasonable for getting off of methadone? They claim to specialize in it.<br />
<br />
Is a 21-28 day detox better, or just dragging it out?<br />
<br />
And does buprenorphine have the magical properties during a short detox that it's supposed to? <br />
<br />
If it would be beneficial, it is feasible to (legitimately) obtain some buprenorphine in order to &quot;extend&quot; the detox another week or so. E.g., if they let him out after being on 2 mg the last day, we could put him on 1 mg. for a 2 days, and then .5 mg. for 3 days, etc., for 10 days or so, if that is advantageous.<br />
<br />
Would it be?<br />
<br />
Would this 10 day detox + a slight outpatient extension be ideal? Or does anyone with experience with this stuff think that a 3 day transfer process + a 7 day taper will be enough to leave without going through withdrawal? Feeling a little unwell -- that's fine. <i>Feeling abject misery, and actual withdrawal symptoms -- i.e., being unable to eat, puking, diarrhea, SLEEP -- that's UNACCEPTABLE.<br />
<br />
So, whatever method we use, we have to avoid having those symptoms once leaving the detox -- they're expensive enough, and we expect he should be thoroughly SEDATED during the process.</i><br />

Please bear in mind: I am not &quot;interested&quot; in outpatient buprenorphine...We tried, and he just cannot accomplish the switch as an outpatient, and we want to get it over with...<br />

Please, anything you guys know about medical detoxes, anything you can let me in on. People don't really give reviews online for detox centers(!), so I need to hear it from anyone hear who got off long-term methadone in a medical detox.<br />

I have been reading these forumns for a while and they have certainly been helpful and now as I start down this next path towards complete sobriety I thought I would start a thread in hopes of support for me and with hopes of providing information and telling my experience to others.<br />

After back surgery in 2004 I started a multi year love affair with opiates.  Towards the end my DOC was  <a href='http://www.drugs.com/methadone.html' target='_blank'>methadone</a> at 50-100mg/day.  That all ended in December of 2006 with an intensive out patient program and a new relationship with  <a href='http://www.drugs.com/suboxone.html' target='_blank'>Suboxone</a>.  I started out with 16mg (I feel certain that was too much) for the first year and then after about a year I went down to 8mg.  I made the switch to the 2mg pills about 8-9 months ago and over that time I have gone from 6mg per day to now 3mg per day which has been my dose for about a month.<br />
<br />
Today I went down to 2mg per day.  1mg at 6:00 AM and 1mg at 6:00 PM and I plan on doing what seems to be the advice of persons on this thread and go down 25% every 4 days.  FYI, I didn't notice the dosage change as I went through my day today - so I guess that is . . . one day down!<br />

Hey Robert_325 from my own very selfish stand piont I hope you reconsider your recent decision.  I have followed your posts for months and have found them very very helpful.  I bet there are hundreds and hundreds of persons, that you have never heard from, that have benifitted from your words.  To tell you that is another reason that I write today for the first time tonight.<br />

I'm having problems starting on suboxone, and looking for advice. I'm not real confident in my doc's abilities at this point, I don't think she's real experienced with suboxone, so that's why I'm here asking questions. <br />

DOC - 11  <a href='http://www.drugs.com/tramadol.html' target='_blank'>tramadol</a> a day, 50 mg <br />

Doctor gave me a script for 2mg 3x a day (with no instructions on what I was to do....thank goodness I've read a bit about the suboxone).<br />
<br />
I waited 20 hours before taking the suboxone, 2mg. Took a second 2mg. Second day, I took 2 + 2 + 2 as prescribed. I felt dizzy and tired most of the day. Finally felt &quot;normal&quot; after taking the last pill in the evening.<br />
<br />
Third day, yesterday, took my first pill and felt tired and light headed. I developed a bad headache and started throwing up. In the afternoon, I ended up taking 4 2mg pills because I thought maybe I wasn't taking enough --it helped my headache, but seemed to make the vomiting and tiredness worse. Evening I took 2 more 2mg pill....for a total of 14 mg.<br />
<br />
I called my doc yesterday and got no answers from her. Today I'm at a loss on how to go forward. What should I take? Back to the 2+2+2?<br />
<br />
I actually don't feel half bad today, but I don't know what I should be taking now.  And I'm afraid to even take a suboxone because that seems to be what precipitated the vomiting yesterday. <br />

I believe I'm at .75 mg -or maybe .65 mg. I know people have systems for tracking this but I don't know for sure. The nice thing is I my w/d 's are very easy to take at this point. Infact as of right now I haven't taken sub for 28 hours. I understand with the half life this is normal. Still I see this as progress cause I couldn't have done this b4. In genreral how much longer before I can jump off?</div>

			<content:encoded><![CDATA[<div>hi all, i have been on a  <a href='http://www.drugs.com/methadone.html' target='_blank'>methadone</a> program for 4 years and every day i wake up feeling withdrawls at some ungodly hour (roughly 5am). i was on 80ml, i am now on 28ml and ive gotten so sick of it, that when i went to see dr rushton at my clinic i got him to change me over to subutex. he gave me 8 8ml tablets and told me to wait 48hrs before i start the transtition, and wait till you get big withdrawls, the only thing is its now half 3 in the morning and i havent had any methadone sinse yesterday at 9am, so thats what, 43 and a half odd hours and i still havent even got the SLIGHTEST symptom of withdrawls, the only thing im feeling is wide awake and cant sleep (hence messaging on a message board at half 3), im confused why im not in withdrawl. so my question is, does anyone think it a bad idea to take my subutex yet?? because i am worried.</div>

It has made work, being with my famile, just being awake extremely rough. My dentist is calling me in some Pain Pills.....Percocets....it is my understanding that these will not even help at all with my paid....Correct? <br />

What can I do if anything?<br />

Also, he sais he could tell from previous X-rays and which tooth it was we will probably be doing a Root Canal tomorrow. Ive heard those hurt quite a bit....never had one. With the Suboxone even allow the &quot;numding&quot; medicine and whatever else they use to actually work? <br />

I've already learned that I shouldn't take  <a href='http://www.drugs.com/advil.html' target='_blank'>advil</a> or  <a href='http://www.drugs.com/claritin.html' target='_blank'>claritin</a> d... what about heartburn and cold meds?<br />

Does everyone gain weight on lexapro? I'm a male and I weigh 280 right now...<br />

I started out with a dose of 5, but will move up to 10 within a week...<br />

Will it make me &quot;happy&quot; or just chill me out or make me numb or remove the anxiety or what?<br />

Will my wife be happier now that I'm on it? In other words, will I be less moody and confrontational?<br />
