Default Specialty Pharmacy and Consumer Driven Health Care

With Consumer Driven Health Care becoming so popular what do you think needs to happen with the coverage of specialty pharmacy. Some healthplans are debating whether to go through Medco or another PBM, or do you think they should find their own SPP/SPV, or to hold off another year as all the projections seem to be fabricated as they come from PBM's. What is your suggestion on SP meds and how plans should acces these drugs for their members. With also keeping in mind that abuse should be avoided (thinking of mail). Thanks for your insight
