And there are many conditions that can cause the type of chronic pain, that cannot be treated any other way, so people are left with narcotic analgesics as their only option.

Neuropathy
Virtually any type of degenerative bone, spinal, or muscular disease.
Cancers
Muscular Dystrophy
Reflex Sympathetic Dystrophy
Multiple Sclerosis
Traumatic Injuries or diseases which have assaulted the body in various ways causing permanent nerve damage.
Phantom Limb pain which can be extremely acute after amputation, and for some, never goes away.
Severe and Degenerative Adult Scoliotic Conditions
Degenerative Disc Diseases
Degenerative Joint Conditions
Migraines caused by unknown factors or injuries that are severely debilitating and limited someone's quality of life.
etc.
Like I said, this is just some of the many conditions that could require long-term narcotic drug use for pain management.
You will find that most people tried all of their other options first, and exhausted them, before resorting to opiod pain management. Very few jumped straight to controlled substances, and as Dave said, you have to have a well documented condition, and have willing cooperated with your doctors, before most of them will resort to prescribing these types of meds.
Personally, I don't see the person who is asking this question as someone wanting methods to seek drugs, I get a completely different impression, it seems to me, though I could be wrong, that he seems to doubt the existence of these types of conditions that could require such a thing.
