Here's the story short:I took care of this patient who was on
heparin gtt at a rate of 1600units/h after a bolus of 7000 in ER.the guy is 195 pounds and his initial PTT was around 28.after 6 hours the routine PTT came back >600.I stopped the heparin,got the lab to redraw him,and they've got the same result.after another 2h the PTT was 68.
now,my problem is that the director of the lab came in in the morning and totally trashed me when I express my doubts about the accuracy of that 600 result.
I'm still fuming here
the patient had no signs of bleeding whatsoever.
I really need some help to understand if there are some conditions that would get the PTT that high,what about giving him the protamine?
