Wellbutrin is a aminoketone, it has little to no effect on serotonin. Any serotonergic effect would be from its suppression of enzymes that regulate metabolism in the liver, much in the same way that grapefruit interacts with many drugs. Doctors quite often prescribe "cocktails" of antidepressants, sometimes it's the only way to get results with treatment resistant patients.
Wellbutrin/buproprion has been known to trigger seizures on its own, though it's not common. Without more information, it's hard to say whether it was the subject's sensitivity to the buproprion, too high a dosage of buproprion, an interaction with something else, what dosage of Prozac her Dr. had her on, etc.
