GENERALLY AVOID: Coadministration with inducers of CYP450 3A4 may decrease the plasma concentrations of sorafenib, which has been shown in vitro to be a substrate of the isoenzyme. Pretreatment with the potent inducer rifampin for 5 days prior to a single dose of sorafenib has been reported to decrease sorafenib systemic exposure (AUC) by an average of 37%. Data are not available for sorafenib in combination with other CYP450 3A4 inducers.

MANAGEMENT: The use of sorafenib in combination with potent CYP450 3A4 inducers such as carbamazepine, dexamethasone, phenobarbital, phenytoin, rifamycins, and St. John's wort should generally be avoided. Although not studied, a dosage increase of sorafenib may be considered if there are no alternatives and concomitant use with these agents is necessary. Close monitoring for sorafenib toxicity (e.g., cutaneous reactions, hemorrhage, peripheral neuropathy) is recommended if the dosage is increased. The dosage should be returned to the normally recommended dosage following discontinuation of the potent CYP450 3A4 inducer. Other known inducers include aminoglutethimide, barbiturates, bexarotene, bosentan, efavirenz, etravirine, nevirapine, and various other anticonvulsants, although the extent to which they interact with sorafenib is unknown.
