Drug interactions between Prevacid and Vfend

ADJUST DOSE: Coadministration with voriconazole may significantly increase the plasma concentrations of omeprazole and other proton pump inhibitors. The mechanism is voriconazole inhibition of CYP450 2C19, the isoenzyme responsible for the metabolic clearance of these agents. According to the manufacturer, voriconazole (400 mg orally every 12 hours for 1 day, then 200 mg every 12 hours for 6 days) increased the steady-state peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of omeprazole (40 mg once daily for 7 days) an average of 2- and 4-fold, respectively, in healthy subjects. In contrast, omeprazole increased the steady-state Cmax and AUC of voriconazole by an average of only 15% and 40%, respectively. The pharmacokinetics of other proton pump inhibitors have not been studied in combination with voriconazole, but a similar interaction is expected.

MANAGEMENT: Patients who are already receiving omeprazole dosages of 40 mg/day or greater should have the dosage reduced by one-half upon initiation of voriconazole therapy. Dosage adjustment recommendations for other proton pump inhibitors are not available. Patients receiving other proton pump inhibitors with voriconazole should be monitored for potentially increased pharmacologic response, including adverse effects, to these agents.
