Interactions between posaconazole and Halfan (halofantrine)

halofantrine and posaconazole (Major Drug-Drug)

CONTRAINDICATED: Coadministration with posaconazole may increase the plasma concentrations of halofantrine, resulting in an increased risk of QT interval prolongation and ventricular arrhythmias including torsade de pointes. The mechanism is posaconazole inhibition of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of halofantrine. Halofantrine has been associated with QT interval prolongation, ventricular arrhythmias, and sudden death, even at recommended dosages.

MANAGEMENT: The use of halofantrine in combination with posaconazole is considered contraindicated.
