Applies to: Nexium (esomeprazole) and Tarceva (erlotinib)
GENERALLY AVOID: Concurrent administration of agents that increase gastric pH, such as H2-antagonists and proton pump inhibitors, may decrease the oral bioavailability of erlotinib and reduce its concentrations in plasma. The solubility of erlotinib decreases when the pH is over 5, resulting in reduced absorption. When coadministered with omeprazole, the area under the plasma concentration-time curve (AUC) of erlotinib decreased by 46% and the maximum plasma concentration (Cmax) decreased by 61%. Time to maximum concentration (Tmax) and half-life did not change. Increasing the dose of erlotinib is unlikely to compensate for the loss of systemic exposure.
MANAGEMENT: The concomitant use of erlotinib with proton pump inhibitors or H2-antagonists should be avoided if possible.
