Interactions between lopid(gemfibrozil) and Colchicine (colchicine)
colchicine and gemfibrozil (Moderate Drug-Drug)
MONITOR: A case of myopathy and rhabdomyolysis has been reported with concomitant use of gemfibrozil and colchicine. A 40-year-old patient developed myalgia, dark urine, anorexia, and elevated liver function tests and creatine kinase after gemfibrozil was added to a long-term colchicine regimen of 0.5 mg every 8 hours. The mechanism is unknown and causality was not clearly determined. Both drugs have individually been associated with rhabdomyolysis, the patient had preexisting risk factors (mild renal insufficiency, chronic liver disease due to hepatitis B, and amyloidosis), and the colchicine dose was high for his level of renal and hepatic function.
MANAGEMENT: Until more information is available, patients should be carefully monitored for response and tolerance. Patients should be advised to promptly report symptoms of muscular pain, weakness, or tenderness and creatine kinase should be measured if such symptoms appear. If myopathy is suspected or if creatine kinase is elevated, the drugs should be discontinued.
