Major Drug-Drug Interaction lithium and duloxetine (Major Drug-Drug)

MONITOR CLOSELY: Lithium may enhance the pharmacologic effects of serotonin reuptake inhibitors and potentiate the risk of serotonin syndrome, which is a rare but serious and potentially fatal condition thought to result from hyperstimulation of brainstem 5-HT1A receptors. The exact mechanism is unknown. The interaction has been reported specifically with selective serotonin reuptake inhibitors (e.g., fluoxetine) but may also apply to other serotonin reuptake inhibitors based on their pharmacologic similarities.

MANAGEMENT: Caution is advised when these agents must be used concomitantly with lithium. Close monitoring is recommended for signs and symptoms of excessive serotonergic activity such as CNS irritability, altered consciousness, confusion, myoclonus, ataxia, abdominal cramping, hyperpyrexia, shivering, pupillary dilation, diaphoresis, hypertension, and tachycardia.
