Interactions between lepirudin and Heparin lock flush (heparin)

MONITOR CLOSELY: Theoretically, the concomitant use of thrombin inhibitors and other anticoagulants may increase the risk of bleeding complications due to additive or synergistic effects on the clotting cascade.

MANAGEMENT: Since heparin is contraindicated in patients with heparin-induced thrombocytopenia, the coadministration of argatroban or lepirudin with heparin is unlikely for this indication. However, if either of these agents is to be initiated after cessation of heparin therapy, clinicians should allow sufficient time for heparin's effect on the activated partial thromboplastin time (aPTT) to decrease prior to initiation of the thrombin inhibitor. If for any reason administration should overlap, it should be undertaken with caution and only after thorough assessment of risks and benefits. Close clinical and laboratory observation for bleeding complications is recommended.
