The active metabolite of leflunomide, known as M1, has been shown to inhibit CYP450 2C9 in vitro and may increase the plasma concentrations of drugs that are substrates of the isoenzyme, including many nonsteroidal anti-inflammatory drugs (NSAIDs) and sulfonylureas. According to leflunomide labeling, M1 caused increases ranging from 13% to 50% in the free fraction of diclofenac, ibuprofen, and tolbutamide at concentrations in the clinical range. The clinical significance of these findings is unknown. Clinical and/or laboratory monitoring may be appropriate for concomitantly administered drugs that have a narrow therapeutic index.
