MONITOR CLOSELY: The coadministration of phenytoin and intravenous immune globulin may lead to hydantoin-induced hypersensitivity myocarditis. One fatality has been reported. The mechanism is unknown and a cause-and-effect relationship has not been definitely established. This interaction may theoretically occur with other hydantoins and immune globulins.

MANAGEMENT: Due to the serious nature of the possible interaction, the clinician should consider monitoring hematologic parameters such as eosinophils as well as blood pressure and ECG tracings if these drugs are coadministered. Patients should be monitored for symptoms such as rash, fever, urticaria, or chest pain.
